Tori's Critters Program Waiver

Participant Waiver and Release of Liability Form 

This form must be signed prior to the visit from TORI’S CRITTERS. 

 

(1) Be aware that animals can and may bite. You may pet and hold animals at your own risk. 

(2) Children must be supervised by an adult at all times. 

 (3) Animals may carry diseases that can be transmitted to humans, proper personal hygiene after interacting with animals is the sole responsibility of the guests. Hand sanitizer will be provided. 

 

I wish to participate in activities involving TORI’S CRITTERS or sponsored by TORI’S CRITTERS LLC. 

I certify that there are no health-related reasons or problems which preclude my participation in this activity. 

I understand that I do so at my own risk. 

I hereby release and waive all claims against TORI’S CRITTERS and any entities affiliated with the foregoing from liability for any and all loss, damage, injuries, claims, demands, lawsuit, expenses and any other liability of any kind, of or to me or any other person, directly or indirectly arising out of, or in connection with my visit or participation in any activity. 

I voluntarily participate in animal related activities. Including but not limited to petting, holding, touching and feeding animals, as well as all interaction and handling of the various animals provided in the program, including but not limited to snakes, lizards, tortoises, insects, rabbits, ferrets , birds, etc. It is agreed on behalf of the event attendee and for his/her heirs, assigns and representatives. I further agree to hold harmless, indemnify and reimburse the Released Parties from and for any sums, costs, or expenses incurred by any of the Released Parties or paid by them to any person (including me or my insurers) in connection with any accident, loss, damage or injury sustained by me or others in connection with my attendance, at or participation in, the above described activity. This means that I will reimburse the Released Parties if anyone makes a claim against them based on damages or injuries I may suffer. Such risks include but are not limited to loose animals, unpredictable event activities that may cause unpredictable behavior from animals, etc. 

Event Attendee FREELY ASSUMES ANY ALL SUCH RISKS, INCLUDING THE RISK OF ANY NEGLIGENCE OF OWNERS. 

I hereby grant TORI’S CRITTERS permission to use my likeness in a photograph in any and all of its publications, including website entries, without payment or any other consideration. I understand and agree that these materials will become the property of TORI’S CRITTERS. I hereby irrevocably authorize TORI’S CRITTERS to edit, alter, copy exhibit, publish or distribute this photo for purposes of publicizing TORI’S CRITTERS. In addition, I waive the right to inspect or approve the finished product, including written or electronic copy, wherein my likeness appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photograph or video.
